Starting times will be assigned to best accommodate player requests and in the best interests of the Club, utilizing the order of entry received with the earliest or requested starting time process as much as possible. Please identify any particular starting time requests (early or late), and we will do what we can.

If you are interested in playing in this event, please return this completed form and payment to be received by no later than August 7, 2010.

Entries received after that date may not be added to the playing roster for this event (you will be notified immediately if we are unable to accommodate you after the entry deadline).
